1. The employee must be employed by a business that is a member in good standing with the Greater Summerside Chamber of Commerce. The employee cannot be an owner, principal or officer of the Company.

2. What is the employees job title and please provide a very brief description of his/her job.

3. Does the employee exhibit exemplary performance in the employee's own responsibilities, as well as service beyond assigned duties? (Please explain)

4. Does the employee offer innovation and creativity to improve the efficiency/effectiveness of the company's operations? (Please explain)

5. How long has the employee been employed on a full-time basis with your business?

6. Does the employee foster cooperation and harmony within the workplace and does he/she promote fairness and respect within the workplace? (Please explain )

7. Does the employee look to represent their company within the community, such as in a volunteer or fundraising capacity? (Please provide examples of their involvements)

8. Has this individual received any other notable achievements? (Please provide copies of newspaper clippings, certificates, etc., if available and/or possible)
