5. Have jobs been created in the community because of the support of this organization/business? (Please explain and provide details)

6. Has the organization/business achieved growth due to its community involvement? (Please provide reasoning or financial support, if available)

7. Has the organization/business earned any other notable achievements? (Please provide copies of newspaper clippings, certificates, etc., if available and/or possible)
